The easy way to print out an order blank is to use the shopping cart and put the patterns, kits, and supplies into the cart.  Then proceed to check out.   After you put your shipping information in, and continue, you will choose your payment method.  Choose "Ordering by Check or Money Order.  You will then see an order blank with all your information that you can print out and send in along with your payment.  You will receive a copy of your order by email.

Postage for the dolls and molds will be at a different rate.
